The Community Health Advocate engages patients in self-care initiatives and helps them navigate the healthcare system to reduce emergency room visits. They provide clinical support, conduct home visits, and connect clients to community resources specifically regarding perinatal and postpartum care.
A high school diploma is required, though a bachelor's degree in a human services field is preferred. Candidates must be bilingual in Spanish, possess a valid driver's license, and have 2-4 years of experience serving underserved communities.

The Harmony Healthcare Long Island is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 4 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. As federally qualified health centers, we serve the individuals in our communities, providing enhanced services, expanded hours and reduced prescription pricing, while raising the level of care. We treat patients regardless of income, residency or immigration status.

The Community Health Advocate functions as a member of an interdisciplinary team to engage patients in self-care initiatives assess for barriers to care and to provide support to patients who need assistance in navigating the health care system. Advocates for and supports the client, engages with community agencies/health care providers and others on his behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/ or hospitalizations. Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions, resources to maintain focus on outcomes and best practices.
This is a non-exempt position.
