We obtained school contracts as well as becoming a Qualified Vendor with the Arizona Division of Developmental Disabilities (DDD). We also contracted with other agencies holding Arizona Early Intervention Program (AzEIP) contracts to deliver services to the birth to 3 population. Essential Duties: Process referrals, schedule initial meetings with families, conduct screenings using standardized protocol and tools approved by AzEIP, and ensure timely delivery of services Assess nature of clients' situations by interviewing and reviewing personal history Ensure proper maintenance of child files, required documentation and the AZ DES automated system Coordinate, attend and schedule assessments; IFSP (6 months and annual), IEP, eligibility determination meetings, transition planning; notify all interested parties of meeting dates and times Communicate client updates to all relevant parties Facilitate referrals to other healthcare professionals and programs Demonstrate knowledge in AzEIP Policies and Professionalism, Child Development, Family Professional Partnerships, Teaming, Assessments and Evaluation, Planning, and Intervention Demonstrate knowledge in AZ DDD/DES policies
A Bachelor's Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education, or a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of coursework and approval by AzDES/AzEIP. Candidates must have a Bachelor's Degree in a relevant field and complete a background clearance. Excellent communication skills and bilingual proficiency in English and Spanish are preferred.
