  • Description: SHS is a nurse-managed FQHC, funded through the Health Resources and Services Administration Bureau of Primary Healthcare; and is a collaborative effort between the College of Nursing and the Sheridan School District. The clinic serves community health needs primarily for residents of Arapahoe, Denver, Adams, and Jefferson counties. Sheridan Health Services is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65%25 of whom identify as Latinx, and 42%25 of whom are best served in another language, with Spanish being the predominant second language. Services offered include primary care across the lifespan; perinatal, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance use disorder treatment. Sheridan Health Services is a Patient-Centered Medical Home and upholds a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. We extend the reach of the clinic by providing services to our community partners in the Sheridan School District and Urban Peak.   • Performed: Provides acute and chronic primary care for the clinic’s pediatric populations Performs the full scope of Nurse Practitioner duties, to include comprehensive physical examination, diagnosis, evaluation & treatment of both chronic and acute illness. Refers to specialists when indicated. Provides culturally & linguistically appropriate services to patients from diverse backgrounds in an underserved, resource-limited setting Coordinates patient care needs with other care team members, including RN Case Managers, Referral Coordinators, Behavioral Health Team members and Pharmacy Team members. Provides individualized client teaching and counseling utilizing current, evidence-based guidelines. Provides health education to patients, students, families, and/or community health partners Collaborates on quality improvement Initiatives under the direction of the Pediatric Clinical Lead and/or Clinical Director Improves and supports care delivery both within the service line and clinic-wide, by participating in programmatic planning, team-based huddles and integrated or all-staff meetings as requested by the Pediatric Clinical Lead and/or Clinical Director Implements and upholds evidence-based policies & procedures Functions as a clinical preceptor and/or clinical instructor for health professions students. Participates in after-hours call in rotation with the other providers on the team Uphold compliance with university and professional policies/procedures. Maintains electronic health record documentation in compliance with accepted standards, in keeping with the policies & procedures of the clinic. Meet rigorous standards for documentation, and complete notes, referrals and follow-ups in a timely manner. Performs other duties as assigned. Maintains all necessary licenses, certifications, credentialing materials and other required documents as outlined by the Office of Clinical and Community Affairs at the College of Nursing.

· Considerable degree of independent judgment and autonomy in practice. · Clinical expertise in pediatric primary care · Willingness and ability to work in an integrated care setting, with a high degree of teamwork and collaboration · Ability to use computer and clinic software, and to document in the Electronic Health Record (EHR) with thoroughness and accuracy. · Exceptional interpersonal and communication skills to work effectively with patients, their families, and other members of the health care team · Excellent knowledge of and ability to perform clinical teaching · Ability to remain credentialed by the health plans and ensure credentialing materials stay current. · Ability to effectively develop and promote practice through participation in marketing and patient recruitment · Ability to take one week-at-a-time on-call in a rotation schedule. · Availability of working evenings and weekends as needed. · Ability to handle average nurse practitioner caseload
