About the role

The Family Support Specialist conducts home visits and provides targeted interventions to strengthen family protective factors and ensure child safety. They manage a caseload, collaborate with community providers, and develop service plans to prevent out-of-home placement.

This position offers a $4,000 sign on bonus

*Paid $1,000 quarterly during your first year of employment. Employees must stay 2 years to keep the full bonus. Employees who leave before 1 year will be required to pay back any bonus provided. Employees who leave after 1 year, but before 2 will be required to pay back 50% of the bonus. Recognizing the value in self care and proper work life balance, Bridges Homeward operates on a 33.5 hour (4 1/2 day) work week. The Family Support Specialist conducts home visits and uses targeted interventions to strengthen family protective factors and resolve conflict. Responsibilities include monitoring child safety, supporting parents in building parenting and advocacy skills, and connecting families to community resources. The role also includes providing case management services and maintaining accurate and timely documentation. This work helps families build safe, stable, and permanent relationships. Manages a caseload of families referred by DCF or other community providers Collaborates with parents, children, DCF staff, Lead Agencies, community resources, and Bridges Homeward staff in developing plans specifying resources and services Provide in home crisis response aimed at stabilizing the family Provide a range of interventions aimed at supporting family capacity to safely care for children in order to prevent out of home placement of children and youth, or to support return of children and youth to their families Coaches Family Time Visitation QUALIFICATIONS FOR A CASE MANAGER Bachelor's degree in human services, social work, psychology, education, or a related field OR high school degree with 5 years related experience

Occasionally provide transportation to clients in your vehicle Bilingual candidates are needed for this position. There is an exceptional need for Spanish and Haitian Creole speaking professionals at this time. Requirements

Candidates must hold a bachelor's degree in a human services field or a high school diploma with five years of relevant experience. A valid driver's license and the ability to provide transportation to clients are required, along with bilingual proficiency in Spanish or Haitian Creole.
