Rehabilitation Therapist

The rehabilitation therapist supports ambulatory clinicians and families by providing consultation on community resources, managing referrals, and participating in appointments to support a youth’s functional level, including housing, academics, and job readiness. Essential functions involve collaborating with the treatment team to evaluate needs, mobilizing resources like housing and academic support, attending necessary meetings, and implementing care plan strategies by linking patients with services.

Candidates must possess a bachelor's degree with major coursework in a related health or occupational field, along with 2-3 years of relevant experience working with children and adolescents concerning mental health concerns, or an equivalent combination of education and experience. A valid new york state driver's license and the ability to meet safe driver credentialing criteria are strongly preferred.
