Case Manager

The case manager will provide direct support, including individual case management, brief counseling, and psychoeducation, while developing, implementing, and monitoring individualized recovery-Oriented service plans for clients in an intensive day treatment program. Key duties involve conducting intake assessments, coordinating referrals to community resources like housing and benefits, and maintaining accurate clinical documentation under supervision.

Candidates must possess a bachelor's degree in a relevant field such as social work, psychology, or counseling, along with a minimum of one year of experience working with adults diagnosed with mental health conditions. Strong documentation skills, basic computer literacy, and the ability to manage a caseload independently and collaboratively are essential qualifications.
