Core responsibilities
The lay health educator will assist with patient health education programs focusing on chronic conditions like diabetes by following physician-Assessed action plans, conducting individual education appointments, and teaching established curriculum classes. Responsibilities also include maintaining accurate records, conducting follow-Up activities, and assessing patient self-Management behaviors and barriers to change.
Requirements summary
Candidates must have knowledge of medical terminology and previous experience as a medical assistant is required, with 2 years of clinical experience as an lpn preferred. Bilingual proficiency in english and spanish is mandatory, along with excellent interpersonal and communication skills, and the ability to work varied hours including evenings and saturdays.
