Core responsibilities
The lay health educator will assist with patient health education programs focusing on chronic conditions like diabetes by following physician-Based action plans, conducting individual education sessions, and teaching established classes. This role also involves maintaining accurate records, conducting follow-Up activities, and assessing patient self-Management behaviors.
Requirements summary
Candidates must have previous experience as a medical assistant and be bilingual in english and spanish, with lpn clinical experience preferred. Required knowledge includes medical terminology, and candidates must possess excellent interpersonal and communication skills, along with the ability to work varied hours including evenings and saturdays.
