Core responsibilities
The lay health educator will assist with patient health education programs focusing on chronic conditions like diabetes by following physician-Assessed action plans, conducting individual education sessions, and teaching established curriculum classes. This role also involves maintaining accurate records, conducting follow-Up activities, and assessing patient self-Management behaviors and barriers to change.
Requirements summary
Candidates must have previous experience as a medical assistant and knowledge of medical terminology, with 2 years of previous clinical experience as an lpn being preferred, and prior teaching experience being an advantage. Required skills include excellent interpersonal and communication abilities, flexibility to work varied hours including evenings and saturdays, and bilingual proficiency in english and spanish.
