CF Care Coordinator

The care coordinator partners with a clinician to support families by connecting them with desired community-Based services and enhancing caregiver executive functioning skills. Responsibilities also include engaging the caregiver-Child dyad in relationship-Strengthening activities to decrease toxic stress and promote child development.

Candidates should be highly organized, self-Motivated, reliable, and flexible, willing to work non-Traditional hours including evenings, and possess an openness to learning and self-Reflection. Preferred qualifications include a bachelor's degree in a related field and a minimum of three years working with diverse families and young children under six.
