Case Manager, Transitional Program & Alumni R

This role involves providing individualized case management, goal-Oriented service planning, and resource coordination for current and former program participants, while also overseeing the growth of the follow-On program for graduates. Responsibilities include guiding participants in independent living skills, supporting the scholarship platform, and providing necessary counseling or crisis intervention for alumni.

Candidates must possess working knowledge of dmv procedures, college registration, legal support systems, and community-Based organizations to effectively connect participants with resources. A bachelor's degree in social science or comparable experience in childcare/Youth residential settings is required, along with the ability to become certified in cpr, first aid, and community water safety.
