Consumer Services Coordinator (Case Management) for San Bernardino School Age

The consumer services coordinator is responsible for coordinating services and supports for persons with developmental disabilities, utilizing person-Centered planning to create and execute individual program plans (Ipps) and conduct ongoing evaluations. This role involves providing advocacy, attending relevant meetings like ieps and itps, seeking generic resources, and managing crisis interventions and incident reporting.

Candidates need a bachelor's degree with one year of relevant experience in developmental disabilities, social work, or a related field, or a master's degree in a related field. A valid california driver's license, reliable transportation, and the ability to maintain good attendance and follow directions are mandatory.
