Consumer Services Coordinator (Case Management) for West End School Age

The consumer services coordinator is responsible for coordinating services and supports for persons with developmental disabilities using person-Centered planning to create and execute individual program plans (Ipps). This involves ongoing evaluation, providing advocacy, attending relevant meetings like ieps/Itps, and ensuring medicaid waiver standards are maintained.

Minimum requirements include a bachelor's degree plus one year of relevant experience, or a master's degree in a related field such as social work or special education. Candidates must possess a valid california driver's license, maintain good attendance, and have good communication skills.
