Wraparound Facilitator for Henry

The facilitator will assist families in developing their wraparound team, consisting of professionals, friends, and family, and create a comprehensive wraparound plan based on the family's strengths and needs. They will also facilitate regular community-Based wraparound team meetings to review the care plan and help families access necessary services and supports to achieve their goals.

Candidates must possess a ba/Bs degree along with at least 24 months of relevant experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. Essential requirements include having reliable transportation, a valid driver's license with car insurance, the ability to pass background checks, and maintaining a designated home office with necessary equipment.
