Wraparound Facilitator for Fayette

The facilitator will help families build their wraparound team consisting of professionals, friends, and family, and create a wraparound plan based on the family's strengths and needs. They will also facilitate regular community-Based team meetings to review the care plan and assist families in accessing necessary services and supports.

Candidates must possess a ba/Bs degree along with at least 24 months of relevant experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. Required prerequisites include having a valid driver's license, car insurance, reliable transportation, the ability to pass background checks, and a designated home office setup.
