Wraparound Facilitator for Henry

The facilitator will assist families in developing a wraparound team composed of professionals, friends, and family, and create a plan based on the family's strengths and needs. They will also facilitate regular community-Based wraparound team meetings to review the care plan and help families access necessary supports.

Candidates must possess a ba/Bs degree along with at least 24 months of relevant experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. A valid driver's license, car insurance, reliable transportation, and a designated home office setup are also mandatory requirements.
