The facilitator will assist families in developing their wraparound team composed of professionals, friends, and family, and create a wraparound plan based on the family's strengths and needs. They will also facilitate regular community-Based wraparound team meetings to review the care plan and help families access necessary services and supports.

Requirements summary

Candidates must possess a ba/Bs degree along with at least 24 months of experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. A valid driver's license, car insurance, reliable transportation, and a designated home office setup are mandatory, and candidates must reside in indiana or within 10 miles of the state line.
