Wraparound Facilitator for Rush

The facilitator will assist families in developing their wraparound team, composed of professionals, friends, and family, and create a comprehensive wraparound plan based on the family's strengths and needs. They will also facilitate regular community-Based team meetings to review the care plan and help families access necessary services and supports.

Candidates must possess a ba/Bs degree along with a minimum of 24 months of relevant experience in clinical, case management, skills building, child welfare, juvenile justice, or k-12 education. A valid driver's license, car insurance, reliable transportation, and a designated home office setup are mandatory requirements.
