Wraparound Facilitator for Howard

The facilitator will help families build their wraparound team composed of professionals, friends, and family, and create a wraparound plan based on the family's strengths and needs. They will also facilitate regular community-Based wraparound team meetings to review the plan of care and assist families in accessing necessary services and supports.

Applicants must possess a ba/Bs degree along with at least 24 months of experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. Candidates must have reliable transportation, a valid driver's license, car insurance, and a designated home office setup, and must reside in indiana or within 10 miles of the state line.
