Family Centered Clinician

Responsibilities include completing strengths-Based assessments, developing individualized treatment plans, and providing in-Home and community-Based interventions in partnership with the family-Centered support specialist. The role also requires coordinating care with external agencies and ensuring timely, accurate documentation.

Candidates must possess a master’s degree in a relevant field such as social work, counseling, or clinical psychology, along with at least one year of experience working with children or youth. A valid driver's license and comfort traveling to various settings are also mandatory.
