Wrap Facilitator for Knox County

The role involves helping families build their wraparound team composed of professionals, friends, and family, and creating a wraparound plan based on their strengths and needs. This person will also facilitate regular community-Based team meetings to review the care plan and assist families in accessing necessary services and supports.

Candidates must possess a ba/Bs degree along with at least 24 months of experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. A valid driver's license, car insurance, reliable transportation, and a designated home office setup are mandatory, and applicants must reside in indiana or within 10 miles of the state line.
