School Transition and Placement Coordinator

The school transition and placement coordinator supports students transitioning from hpds to their home community schools by coordinating educational placements and necessary supports. This role also involves organizing graduation activities to celebrate student achievements.

Candidates should have a bachelor's degree in learning disabilities, special education, or social work, with a master's preferred. A minimum of three years of experience working with children with learning disabilities is preferred, along with strong organizational and communication skills.
