Transition Specialist

The transition specialist uses professional judgment and best practices to deliver quality case management services, assisting youth transitioning out of foster care to achieve their goals. Key functions include assessing client needs, obtaining resources, advocating with various systems, providing crisis intervention, developing service plans, and maintaining accurate documentation.

A bachelor's degree in human services, social work, education, or a related social science field is required, along with two years of experience in case management, job development, or crisis intervention with youth. Candidates must also maintain required certifications like first aid and cpr, possess a valid texas driver's license, and pass pre-Employment screenings.
