Community Based Services Provider

Provide direct 1-1 teaching, coaching, and rehabilitative counseling to youth struggling with mental health and developmental challenges in their homes and community settings. Maintain accurate documentation of services in electronic health records and advocate for the youth's needs with other providers and family members.

Requires a bachelor's degree in social work, psychology, or a related human services field plus two years of applicable experience. Candidates must possess a valid new york state driver's license, a reliable vehicle, and the ability to work flexible hours.
