PRC Case Manager (Mid-Hudson Region - New York)

The case manager manages a caseload of families, ensuring the program meets performance targets while providing face-To-Face support and documentation. They also participate in community outreach and assist in the delivery of educational events and family engagement activities.

A bachelor’s degree in social work, psychology, or human services is required, along with experience in case work or human services. Candidates should have experience with at-Risk families and knowledge of community resources.
