Family Engagement Specialist, School-Based Mental Health

The specialist will act as a resource for students and families by conducting needs assessments, linking them to community-Based services, and addressing social determinants of health impacting youth outcomes. Duties also include providing children’s health home care management after-Care for qualifying youth and developing home-Based crisis and safety plans.

A master's degree in a human service field is preferred, but a bachelor's degree with at least two years of experience in service planning or care coordination is acceptable. Candidates must possess strong integrity, advocacy skills, organizational competence, the ability to navigate ehrs accurately, and must have a valid nys driver's license and reliable vehicle for local travel.
