Job description

Overview Provides supports to the people we serve in community and facility based employment activities.

Responsibilities

Primary duties and responsibilities include but not limited to: Supports the development and ongoing revisions to Individual Habilitation Service Plan by providing input and information. Tracks individual’s progress towards goals and objectives in Staff Action Plan. Segments job duties and assignments into specific steps to facilitate the learning of the people we serve. Monitors individual’s level of performance including determining when support can be faded. Monitors the quality of the work that the people we serve produce in order to ensure it adheres to industry standards and all tasks are completed within an acceptable time frame. Serves as a liaison between employee and employer and facilitates appropriate worker to supervisor relationship. Maintains accurate records on job coaching services provided to each person we support. Documents and recommends to supervisor any action necessary to overcome employment barriers. Responds to crisis in accordance to policy and training. Develops and maintains written documentation, as requested. Makes all decisions based on agency vision and values. Performs all additional related duties as requested by supervisor and/or administration.

Qualifications

High School diploma or GED and six (6) months of employment experience required. Good math and communication skills. Must maintain a valid NYS driver’s license throughout employment. Must have reliable means of transportation. Physical aspects will include being able to bend, push, pull and lift a minimum of 35 lbs.
