The Personal Assistance Services and Supports (PASS) works in collaboration with the clinical consultants and PASS families to support their children with special health care needs with daily life skills, according to their collaborative developed behavioral plans. The direct support provided through PASS objectively is to assist children in developing skills such as Accomplishing activities of daily life, Keeping safe & making good decisions, Learning social roles & skills, in home and community-based support systems to participate and function in the community and in home to ensure each PASS family is supported to help their child succeed in the least restrictive and most natural setting.
Responsible for the direct provision of personal assistance and support services to PASS families. Works collaboratively with clinical consultants and PASS families to support children with special health care needs. Implements direct personal assistance goals for a child developed by the family in conjunction with the agency. Assist with child’s ability to participate in home and community activities that require peer interactions and use of social skills for rewarding social interactions. Assist with child’s ability to self-regulate to maintain safety. Assist with child’s social and communication skill development. Assist in the child’s ability to remain at home, maintain activities of daily living, participate in the community, and transition into young adulthood. Providing information and all training to PASS families on service plan goals specific to their child. Provide recommendations to work toward child’s and family's goals. Writing organized and in-depth daily contact notes and keeping data collection on service plan goals, in a timely manner (data, timesheets and incident reports). Drives children to destinations, as approved by parent/caretaker and in conjunction with their PASS goals Shift duties will vary depending on the specific needs of each The PASS Families accordance to child’s needs. child.
