Job Summary & Deliverables The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure to ensure excellence in client care, fiscal management, and regulatory compliance. Provides therapeutic behavior management as prescribed by care plan Assists therapist as needed to develop and review safety/crisis plans and care plans Coordinates services among internal/external agencies and internal/external providers Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly Ensures continuity of care Meets required productivity standards and other organizational expectations
Education Bachelor’s degree in Social Work or related field Experience Internship experience as required for SSW license Licenses/Certifications SSW licensure in state of practice CPR certification Valley de-escalation certification
