  • Range: $23.50 - $28.00 HourlySummary The Intensive Care Coordinator (ICC) is responsible for coordinating all services and support identified in the youth’s Individual Care Plan (ICP). They are responsible to the Senior Care Coordinator, CSA Assistant Program Director and the CSA Program Director. Bachelor's Degree starts at $23.50 per hour / MA Degree starts at $28.00 per hour Child and Family Services recognizes the power in Diversity and believes that Equity is a Human Right. $1 per hour Language Differential! (eligible languages include: Cape Verdean Creole, Haitian Creole, Portuguese, or Spanish) Child & Family Services carefully considers a wide range of factors when determining compensation including, but not limited to, prior experience, education, certification(s), license(s), skills and expertise, location, internal equity and other factors that are job related and consistent with business need. Our goal is to support, reward and compensate the entire individual. Depending on role eligibility, your offer may include a bonus or other incentives. Job Responsibilities & Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Completes initial and ongoing training on the use of Wraparound and how it applies to the work that is done with families. Completes intakes with families once assigned in alignment with provision of service requirements set by MassHealth. Develops a risk management/safety plan in collaboration with the family and collaterals. Completes a comprehensive, home-based assessment inclusive of the CANS Assessment and other tools as determined necessary, in collaboration with the family and other providers, which occurs in the youth’s home or another location. In collaboration with the family and the Family Partner (when applicable), identifies who the family would like to be part of the Care Planning Team (CPT). Develops and implements a youth- and family-centered Individual Care Plan (ICP) in collaboration with the family and other collaterals. Facilitates ongoing Care Plan Meetings with the family and other team members. Monitors, reviews, and updates the ICP to reflect the changing needs of the youth and family. Makes referrals to community-based support and services as determined by the family at the Care Plan Meeting. Follow-up on referrals is expected. Maintains regular contact with the family, youth (where appropriate), and other relevant persons in the youth’s life (collaterals). This occurs in a method preferred by the family but is primarily delivered in-person, face-to-face. ICCs are required to meet families at least twice a month in-person. Identifies natural supports available to the family, in collaboration with the family and the family partner. Provides family education, advocacy, and support to the family. This is accomplished through means such as assisting with system navigation, supporting and helping the family with the education system by attending school meetings and participating in the development of any school plans/IEP’s; and within other systems such as the courts, state agencies and community organizations. Identifies and actively assists the youth and family to obtain and monitor the delivery of available services including medical, educational, social, therapeutic, or other services. Provides after-hours phone support to address urgent child or family crises. Arranges or provides home visit support services after-hours as needed to address urgent child or family crises. Utilizes mobile crisis as determined necessary. Maintains regular communication with the Care Plan Team members and updates collaterals as needed. Coordinates services with state agencies, the school system, primary care, and other service providers as indicated on the ICP. Prepares all necessary reports and documentation of services, including authorizations, Individual Care Plans, and evaluation data. Engages in robust discharge planning, assisting the youth and family in assessing and accessing other levels of care as clinically appropriate. Performs other duties as required. QualificationsRequired Education and Experience Care Coordinators must be Master’s level or Bachelor’s level, be experienced in navigating any of the child/family–serving systems and have experience advocating for family members who are involved with behavioral health systems. Associates or High School Diploma require 5 years’ experience working with the target population. Care Coordinators must have a valid Massachusetts driver’s license and an automobile. Care Coordinators must be certified in the Massachusetts CANS. Availability during evening/weekend hours is required.
