Case Manager - 4961

The case manager will provide integrated counseling, develop comprehensive individual service plans (Isps) with participants outlining goals, and assist in developing essential skills like study habits and time management. Responsibilities also include tracking all interactions in the dycd participant tracking system and facilitating workshops focused on social services and resource navigation.

Candidates must possess a bachelor's degree in social work or a related field, or an associate's degree with completion of family development training, along with at least three years of experience in case management or counseling involving city systems. Experience working with public assistance, child welfare, education, and housing systems is specifically required.
