Community Coach (Per Diem) [Lawrence L. Frank Center]

As a community coach, you will guide individuals with developmental and physical disabilities, focusing on skill-Building and community integration. You will also develop individual service plans and maintain relationships with families and agency staff.

A high school diploma or ged is required, along with a bachelor's degree or equivalent experience. Experience working with individuals with disabilities and case management services is also necessary.
