Transition Specialist

The transition specialist supports individuals through structured, goal-Based activities to promote independence, employability, and community involvement, including teaching daily living and employability skills. Responsibilities also involve supporting participants at community job sampling sites and maintaining required program documentation.

Candidates must possess a bachelor's degree in human services or a related field and hold a valid nj driver's license with limited points. The role requires strong communication skills, the ability to work independently and collaboratively, and comfort in building community relationships.
