Clinical Care Coordinator

The clinical care coordinator provides home-Based services to families facing various difficulties, working with a licensed clinician to implement therapeutic interventions and community services to promote family wellbeing. Primary duties involve utilizing a strengths-Based model to assist families involved in child removal avoidance or reunification processes, assessing needs, and connecting them with state resources.

A bachelor's degree in social work or a related field is required, along with a minimum of one year of experience in a similar role, preferably in child welfare, parent education, or home-Based services. Candidates must possess strong organizational and critical thinking skills, basic computer proficiency, and the ability to travel statewide, holding a valid driver's license.
