Social Worker

The social worker will provide home and community-Based early intervention services to infants and toddlers at risk for developmental delays, partnering with families to ensure individualized, evidence-Based service delivery. Responsibilities include participating in multidisciplinary assessments, developing individual family service plans (Ifsp), providing direct intervention, and acting as a service coordinator for the family.

Candidates must possess a master's degree in social work from an accredited school and be a certified or licensed social worker as required by massachusetts, with experience working with infants, toddlers, and families being preferred. Strong skills in communication, documentation, knowledge of family systems, and reliable transportation are mandatory.
