Bridges Liaison

The bridges liaison is responsible for determining eligibility, completing assessments, developing individualized bridges plans, and coordinating housing and care management needs for young adults transitioning to adulthood. This role involves direct support, teaching independent living skills, and ensuring participants are connected to necessary community resources for self-Sufficiency.

Candidates must possess a bachelor's degree in social work, human services, behavioral health, or a related field, along with a minimum of two years of experience working with families in the child welfare system. Proficiency in standard computer software and the ability to maintain reliable personal transportation are also required.
