Day Program Facilitator

The day program facilitator will transport clients to and from activities and support them in community settings. They will implement individualized service plans and ensure the safety and well-Being of each individual.

Experience working with individuals with developmental disabilities is preferred, but training will be provided. Candidates should have strong communication skills and a commitment to person-Centered care.
