The Social Worker provides evaluation, determination, and case management services for students, including the creation of IEPs and 504 plans. They offer counseling for students with trauma or mental health needs and coordinate support between families, staff, and community resources.
Candidates must hold a valid New Jersey School Social Work Certification and a Bachelor's degree. Experience working with underserved populations in a school setting is required.
The ideal candidate will have experience working with children and families in a school setting, be able to provide support to students with various needs, and coordinate with teachers and other school staff to ensure that students receive the best possible support. As the Social Worker of a Child Study Team they will serve students enrolled at CCSP. ● Provides Evaluation and Determination services in accordance with New Jersey State Code and Guidelines. Case Management involves the IEP/504 creation process. ● Provides social assessments and reports as needed for students receiving Initial evaluations or Reevaluations. ● Conduct & Coordinate assessments and evaluations to determine the needs of individual students. ● Conduct meetings during school hours. ● Supports the coordination of SEMI reporting and related services. ● Attend Assessment Plan meetings for all students referred for Initial evaluation ● As assigned, completes social assessments, creates reports submitted to the Child Study Team Manager per Achievers ECP procedures ● Maintain a caseload in compliance with state timelines and other mandated state/school guidance. ● Complete Case Management responsibilities as assigned ● Attend Eligibility Conference for Initial and Re-evaluations if requested ● Work in compliance with the Catapult Learning Policy and Procedure manual for Evaluation; Determination services in New Jersey ● Expert knowledge in NJ Code and procedures regarding student services and accommodations ● Counsel students with IEPs ● Support attendance team ● Screen for signs of trauma and counsels students with trauma and other mental health needs ● Other responsibilities as assigned ● Work closely with school administration, teachers, and other staff members to identify students who need additional support. ● Develop and implement Individualized Education Plans (IEPs) for students with special needs. ● Provide support to students with behavioral, emotional, and academic challenges. ● Facilitate communication between school staff, families, and community resources. ● Provide crisis intervention and support to students and families as needed ● Maintain accurate and up-to-date records of student progress and services provided ● Attend meetings and trainings as required Essential Qualifications: Bachelor's degree Ability to work independently and remain focused on task completion Excellent ability to interact and communicate with students of all ages, parents, and school personnel Strong written communication skills Excellent motivation and ability to complete a multi-step process involving many participants Excellent organizational skills Computer access and literacy to be able to use relevant web-based software Proactive; problem-solver; and self-driven Good organizational and time-management skills Valid NJ School Social Work Certification Experience in Social Work for an underserved population Compensation & Benefits Salary range (65k - 85k) based on experience and certification Comprehensive benefits package Instructional coaching and ongoing professional development Leadership and growth opportunities within a high-performing school
