Core responsibilities
The coordinator provides care coordination for youth with complex behavioral health needs by partnering with families to develop safety plans, build child and family teams, and link them to necessary resources and services to achieve goals. This involves designing and delivering services that meet youth needs within the high fidelity wraparound tier of pathways to success.
Requirements summary
Candidates must possess strong organizational, time management, and excellent interpersonal/Engagement skills, along with the ability to flex schedules as needed. A bachelor's degree in a related human service field or an illinois certified recovery support specialist certification plus two years of supervised experience is required, along with meeting dcfs/Cants clearance and being at least 21 years old.
