Core responsibilities

The lpn/Rn will collaborate with the health services coordinator to deliver consultation nursing services to clients across various programs, acting as a nursing expert to guide staff in implementing high-Quality healthcare services. Responsibilities include identifying and monitoring client health needs, implementing care priorities, educating stakeholders on health topics, and communicating with physicians regarding client status and recommendations.

Requirements summary

Candidates must possess a current, valid licensed practical nurse or registered nurse license eligible for practice in pennsylvania, with rn preferred for night shifts. Experience working with adults with intellectual/Developmental disabilities, mental health diagnoses, and/Or tbi is preferred, and lpns must have one year of behavioral health experience for night shift consideration.
