Ohio Rise Care Coordinator

The ohio rise care coordinator provides family-Centered and community-Based wraparound services, developing care plans and coordinating teams based on individual needs. They conduct assessments and ensure services are delivered according to the care plan.

Candidates must have a bachelor’s or master’s degree in social work, counseling, or a related field, along with a minimum of three years of experience working with children and families. They should also possess skills in managing complex cases and be culturally responsive.
