Community Integration Coach - Hartford (Full and part-time)

The community integration coach establishes life-Changing relationships with participants to optimize their independence, rehabilitation, and recovery by facilitating communication, advocating in the community, and teaching daily living and social interaction skills. Key duties include supporting clients transitioning from inpatient settings and serving as a positive role model.

A high school diploma or ged is required, along with experience working with psychiatric disorders or co-Occurring populations and familiarity with trauma-Informed care. Candidates must also possess good computer skills and have a valid driver's license with a reliable, insured vehicle.
