Job description
- CHD (Center for Human Development) is searching for a Community Integration Coach in Hartford, CT at our CTLP (Community Transitional Living Program) residential program. Our residential and coaching staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation and recovery. CHD (Center for Human Development) is searching for a Community Integration Coach in Hartford, CT at our CTLP residential program. Our residential and coaching staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation, and recovery. Responsibilities of the Community Integration Coach include, but are not limited to, facilitating communication, advocating in the community and with other providers, supporting/teaching activities of daily living skills, being a positive role model, helping transition the client from an inpatient setting to living in the community, and teaching social interaction skills. REQUIRMENTS:
- High School diploma or GED required
- Experience working with psychiatric disorders or co-occurring population
- Experience with trauma informed care and ability to understand phycological diagnoses
- Ability to advocate for client in community settings
- Good computer skills
- Valid driver's license and reliable, registered, and insured vehicle
- Approved criminal and driving background record check

Open schedules
- Full-time, 10:00am - 6:00pm.
- Days on: Tuesday, Wednesday, Thursday, Friday, Saturday.

- Part-time: 10:00pm - 6:00pm,
- Days on: Sunday, Monday.
