School-Based Community Support Worker

The community health worker will provide care coordination to youth and families, assisting them in accessing telebehavioral counseling and various offsite community resources. They will also conduct comprehensive screenings, provide follow-Up support, and assist with necessary documentation like referrals and consents.

A high school diploma or equivalent is required for this role, along with strong computer skills, particularly with electronic medical record databases. Candidates must also possess knowledge of local community agencies, organizational skills, and strong interpersonal and communication abilities.
