Core responsibilities
The care coordinator ii is responsible for coordinating multiple aspects of the youth’s and/Or families’ care, providing high-Quality psychosocial care, and working with providers to meet client needs. Essential functions include assessing needs, implementing strength-Based service plans, coordinating resources, and delivering psychoeducational training.
Requirements summary
A bachelor's degree and a minimum of 18 months of relevant experience are required for this role. Candidates must also possess an unrestricted, valid nys driver's license for at least one year with a clean record, and cpr certification is required or must be obtained within 60 days for the youth act program.
