Wrap Facilitator for Noble County

The role involves assisting families in developing a wraparound team composed of professionals, friends, and family, and creating a comprehensive wraparound plan based on the family's strengths and needs. The facilitator will also help families access necessary services and supports to achieve their goals and lead regular community-Based team meetings to review the care plan.

Candidates must possess a ba/Bs degree along with a minimum of 24 months of experience in clinical work, case management, skills building, child welfare, juvenile justice, or k-12 education. Essential requirements include having a valid driver's license, car insurance, reliable transportation, the ability to pass background screens, and maintaining a designated home office setup.
