Core responsibilities
The clinical preceptor assists in clinical orientation, validates competencies for new and current staff, and provides student observation experiences while also delivering direct patient care and case management for a diverse community-Based caseload. Responsibilities include facilitating transitions from orientation, tracking progress, conducting competency assessments, developing patient-Centered plans of care, and ensuring compliance with regulations.
Requirements summary
Candidates must be a graduate of a state board approved registered nurse program, possessing a minimum of 2 years of home health or hospice case management experience. Required certifications include a valid driver's license, current rn license, and obtaining cos-C within 12 months if in home care, along with bls and mandatory training completions.
