Core responsibilities
The case manager will actively engage with young adults, ages 17-21, preparing for community transition, serving as a positive role model to develop socially effective values and behaviors. Responsibilities include supporting residents in developing money management, employment, and daily living skills, while contributing to transitional living plans and assisting with administrative duties.
Requirements summary
Candidates must possess a bachelor's diploma in any field plus two years of case management experience with children and families, or a human service bachelor's degree with preferred case management experience with children/Youth. Alternatively, a human service master's degree with prior student placement experience in case management for children/Youth and families is acceptable.
