Registered Nurse--Diabetes Educator

The registered nurse collaborates with providers to offer patient-Centered care to high-Risk diabetes patients, primarily through telephonic follow-Up to empower self-Management using education and technology. Responsibilities include conducting patient outreach, assessing needs, establishing care plans, utilizing digital tools for remote monitoring, and coordinating care transitions.

Candidates must be a licensed registered nurse in rhode island or hold a multi-State compact license, with a minimum of three years of direct patient care experience, including diabetes management. Obtaining cdces or cdoe certification is required within one year of hire, alongside proficiency in electronic health records like epic and strong clinical judgment.
