Bridges Liaison

The bridges liaison is responsible for determining eligibility, completing assessments, developing individualized bridges plans, and coordinating housing and care management needs for young adults transitioning to adulthood. This role involves direct service provision, including monthly face-To-Face meetings, court appearances, and teaching independent living skills.

Candidates must have completed a post-Secondary program in social work, human services, behavioral health, or a related field, along with two years of experience working with families in the child welfare system. Proficiency in standard computer software and possessing reliable personal transportation with adequate insurance are also mandatory requirements.
