Community Coach (Part-Time) [Monday-Friday (2:00pm-6:00pm)] [Claremont]

The community coach will guide individuals with developmental and physical disabilities in the community, focusing on fostering independence and skill-Building aligned with person-Centered plans. This involves coaching for community integration, building supportive relationships, advocating for participants, and developing/Implementing individual service plans.

Candidates must possess a high school diploma or ged and a valid driver’s license, along with 12 ece college units or equivalent qualifying courses. Required experience includes 1-2 years in case management services and 1-2 years in planning/Implementing activities for individuals with disabilities, alongside proficiency in conflict resolution and strong computer skills.
